You will need to fill out all enclosed forms. The forms included are: the health history, demographics, and HIPPA. Most patients find the paper work challenging. We recognize that it is time consuming and may appear complicated; however, the health history is an important part of your initial visit. We recommend you begin early looking over and filling out the forms. You may want to ask a friend to help you. We realize that paper work can be considered daunting, and we appreciate you cooperation in this matter.
- I have completed the health history.
- I have completed the demographic forms.
- I have completed the HIPPA form.
Test Results
- I have compiled all pertinent test results for Dr. Neville’s review.
Directions
- I have familiarized myself with the driving directions including route and distance and allowed extra time for unexpected delays.
Cancellation
- I am aware that there is a 24-hour cancellation policy.
